Authorizations and Additional Forms
REQUIRED Authorization / verifiation and gift forms
These documents support different loan products and are available as needed.
Credit Card Authorization Form
Authorizes The Lender AI to charge borrower for Floor Cert, SS89, WVOE, ect.
Verification of Rent or Mortgage (VOR/VOM)
Authorizes verification of the borrower’s income and employment as part of the loan process.
Verbal Verification of Employment (VVOE)
Must be provided and dated within 10 days of closing for all loans.
Lender AI Gift Letter
Gift letter must be fully executed and match verified funds from title at closing and/or earnest deposit.
Wholesale Submission Form
Required for all new loan submissions. Data entered on from must match what is being submitted in Lending Pad. Form must be uploaded into Lending Pad under the "Submission Forms" folder.
